Yup, Hospitalists and Intensivists, and most hospitals have an Infection Prevention team who takes point on such matters in terms of response and teaching lessons learned.  There are many cohorts comprising modern American medical treatment...much of it created out of necessity for a population largely unable to understand how to care for itself...like everything, good and bad about it, but still better than going to Cuba...though idjits trying to bring Cuba here.

I once read or heard something about Vietnam. it was said that the intensive trauma experiences provided doctors with a unique learning experience. It was said that the learning saved more lives later on than were lost in Vietnam. I am not saying it is true.

Some RNs perform very specialized jobs.

He jacked up my hospital bed maybe 5 feet off the ground, created a sterile field kinda, draped me in paper or cloth. He lay my arm out on my food tray, also covered. The he joked about putting on his surgeon costume including his face mask. He inserted the PICC line in a few minutes while in my room. Amazingly efficient. No pain. He used ultrasound and other electrical sensing stuff on my chest to locate the delivery. I think he sent electrical signals through the guidewire.

Then a portable xray guy shows up and takes an xray then a radiologist somewhere says yup it is in the right spot then I am out the door in maybe 30 min after the start. The RN was maybe 55 years old.I never saw an MD for this part.

Then hospital outpatient for 2 weeks to have ertepenem antibiotic delivered IV as it is covered under medicare part b if done there rather than at home.

The IV port is under my right arm and the tip is down most of the way to my sternum.

I am a geek and this interesting.

FYI -

PICC lines are inserted by:

Board certified radiologists trained in vascular interventional procedures
Qualified and specially trained radiology nurses
Radiology physician assistants
Radiology nurse practitioners

I am sure my RN was specially trained.  He used ultrasound (US) to distinguish the vein (good)  from the artery (bad)  in my arm. He numbed the area around the insertion point. He inserted a large needle maybe an introducer.  He had laid a large T shaped device with LEDs on my chest. I think it picked up electrical signals from the guidewire. It indicated if the guidewire went too far across my chest rather than down into my vena cava. I think he advanced the PICC line over the guidewire.

When he was satisfied he calls the portable X ray tech who shows up and takes the X ray. Then they call a radiologist somewhere who views the Xray somehow and says the PICC line is in the right location. I was impressed by the rapid team work. Specialized RNs have taken over many jobs like this.  I may have left out some steps. The RN brought a large cart full of devices with him.

You're like a Buick on a lift...technology keeps getting better and government/peer/union regulation so endemic...only qualified staff doing things...that and fear of malpractice and negative press makes sure no yahoo will be allowed to go near a patient.

She heard oral arguments today via telephone from her hospital bed. She is having an ERCP procedure to take a stone out of her bile duct.  I listened to her Monday. She sounded very lucid. I do not like her opinions.

I listened to one of the SCOTUS hearings. I was surprised that Ginsberg was really sharp. I do not agree with her but she was lucid. I expected much less. Sotomayor came across like a dullard IMO.

RoboSota is dull because she is a puppet...she's there to vote as instructed by the likes of Ginsberg more than any other, Kagan is at best a half and half (puppet/advocate) as is Souter...Ginsberg isn't dumb...she knows exactly what she wants to happen and works to that end, knows she is there to re-write the Constitution not adhere to it...which is why the DemoCommies are crapping their pants if she cacks or resigns...the leftist coalition will have fractured leadership that is likely to be less influential before even considering the impact of a replacement...which is why Ginsberg refuses to resign and hang on to the last gasp.  And it will be a last gasp because unless the greatest larceny in earth's history unfolds there is no way Trump should lose to Biden or whoever they may game to replace him with (the latter frought with alienating one leftist tribe or other)...so her hopes of hanging on until a fellow DemoCommie enters office and allowing her to step aside seems pretty unlikely.
