You would think it must have rapidly lost cabin pressure.  I'm guessing there are alarms that warn of dropping pressure, that would give the pilot time to either engage an emergency oxygen supply or descend to a lower altitude...unless it all happened very rapidly.

Frankly I don't think I'd want to go any higher than 10,000 feet in a small plane.

Over 10,000 and higher hypoxia and decompression becomes much more an issue.  Thus I think most small craft fly lower either by design of their cabin pressure tolerance or airspace restrictions.  Typically commercial air is at 28-35k, military has the higher altitudes and civilian the lower.  In the lower band high-end private jets will be higher and the small single-engine props lower.
